This 15-year-old male presented with complaints of gradual onset diplopia and blurred vision in both eyes. Corrected visual acuity was 20/25 with -2.00 spectacles. When asked about the diplopia, the patient describes it as being both vertical and horizontal, with the images separated obliquely (both up-and-down and side-to-side). What do you think could be going on?

Why there is a complaint of blurred vision in both eyes?
Perhaps there’s an underlying uncorrected refractive error. Of course vision improved to 20/25 with adequate correction
I would have liked to see the photo of the primary position without head tilt. Anyway I love this clinical case and usual, thank you very much.

But I honestly thought double Maddox rod is indicated when patient has bilateral CN IV palsy. Thanks again & again!!
