A 12-year-old male reports a one day history of “blurred vision.” No pain, history of trauma, or other complaints. He states that his vision seems “cloudy.” He lives with his parents, two siblings, and multiple cats and kittens. His exam is significant for a visual acuity of 20/20 in the right eye and 20/40 in the left. External and slit lamp exams are otherwise unremarkable. His dilated fundus examination and OCT images are shown above.

  1. How can you say “papilledema” in neuroretinitis, shouldn’t it be disc swelling.
Isnt papilledema reserved as a term for increased ICT only?

    • Yes, you would be correct. The fundus photo has the “appearance of papilledema”. However, if one has verified that the intracranial pressure is normal by LP with opening pressure recording, then this disc finding would be better described as “disc swelling”
