A 66-year-old male presents with redness and blurred vision in his right eye for several months. Past ocular history is significant for LASIK surgery in the right eye 12 years ago, after which he developed an infection, eventually requiring LASIK flap removal and prolonged treatment with antibiotics. He was left with a large scar and underwent penetrating keratoplasty (PKP) in the right eye approximately 6 months ago. He is currently on topical antibiotic (moxifloxacin) and steroid drops four times daily. Vision is 20/400 in the right eye and 20/25 in the left eye. Intraocular pressure is normal in both eyes. Pupil exam is normal. Funduscopic exam is also normal. Slit lamp exam is shown in the photo below:

Chronic rejection or infection both viral and bacterial infections are likely possibilities.

Graft rejection
Delayed graft resection
One can put differential diagnosis: herpetic keratitis, graft failure and stem cell deficiency
Lamellar keratoplasty has been done, as appears from picture
this is herpetic ulcer with stromal keratitis
Microsporidial infection
Endothelial graft rejection
Diffuse lamellar keratitis
Mycobacterium infection
Could be a flab rejection
Graft rejection delayed
