Surgery: Anterior Transposition of the Inferior Oblique Muscles

This child presented with inferior oblique overaction in both eyes and Dissociated Vertical Deviation or DVD, which was more pronounced in the right eye than in the left eye. Asymmetric transposition and anteriorization of the inferior oblique was performed in both the eyes. The inferior oblique transposition in the right eye was 3mm in front of the inferior rectus muscle to address the larger amount of DVD in that eye. The inferior oblique transposition on the left eye was to the zero position (adjacent to the insertion of the inferior rectus) because there was less DVD in the left eye.
