So we’ve given this patient topical anesthesia, but she’s still not lost movement.

I did the rhexis with the needle.

I like to make a a small opening and go upwards first by about two millimeters and then turn the flap, And I don’t ever let the flap unturn because wherever the flap edges are, I know if I continue in that direction, I will not go anywhere except in that direction, which is how you have that perfect central capsulorhexis.

And it also maintains the curve. So the CCC, which is the continuous curvilinear capsulorhexis, needs to continuously go around.

So always fold down the entire flap of the capsule even if it’s segmented like this.

And then wherever the flap is folded, it’s gonna guide where your rhexis is going to go.
