Quiz: 81-Year-Old With a Progressive Decline in Visual Acuity

An 81-year-old male presents with a progressive decline in visual acuity in both eyes over the past 18 months. The patient has a known history of neovascular age-related macular degeneration (nAMD) in both eyes and has been undergoing intravitreal anti-VEGF (bevacizumab) injections every 7-8 weeks for the past 3 years.

On examination, uncorrected visual acuity is 20/200 in each eye, with no improvement upon pinhole testing. Slit-lamp examination reveals clear corneas, normal anterior chambers, and bilateral 1+ cortical cataracts. Fundus examination demonstrates a cup-to-disc ratio of 0.3 with evidence of mild neovascular fibrosis in both eyes.
