A 48-year-old man presents with decreased vision in his right eye for 1 month. He reports a prior history of episodic redness, watering, and photophobia in the same eye. There is no history of trauma. Best-corrected visual acuity is 6/60 (20/200) in the right eye and 6/6 (20/20) in the left eye. Examination reveals stromal scarring with vascularization and severe central corneal thinning. Corneal sensation is tested and found to be reduced.
